CTI HAS ADOPTED THE FOLLOWING POLICIES FOR CHARGES FOR HEALTH CARE SERVICES
We will charge persons receiving health services at the usual and customary rate prevailing in this area. Health services will be provided at no charge, or at a reduced charge, to persons unable to pay for services. In addition, persons will be charged for services to the extent that payment will be made by a third party authorized or under legal obligation to pay the charges.
We will not discriminate against any person receiving health services because of their inability to pay for services, or because payment for the health services will be made under part A or B of Title XVIII (“Medicare”) or Title XIX (“Medicaid”) of the Social Security act. We will accept assignment under the Social Security act for all services for which payment may be made under part B of Title XVIII (“Medicare”) of the act. We have an agreement with the State agency which administers the State plan for medical assistance under Title XIX (“Medicaid”) of the Social Security act to provide services to persons entitled to medical assistance under the plan.
FEE AGREEMENT
CTI’s contractual obligation is to render screening, crisis intervention, counseling, case management and medication clinic services to individuals when the agency is capable of providing that service.
It is important that you understand:
If you are capable of paying for services, you are expected to do so. You will be charged according to your ability to pay, based on your family income, number of persons in the family, etc.
A sliding scale fee (reduced fee) can be set only with verification of your income. Insurance, Medicaid, Medicare or certain government contracts may pay for or all of your services. You are expected to help CTI collect payment from third parties by bringing information or forms from your insurance company and assigning that insurance payment to CTI. If you obtain insurance or your insurance changes, please notify us immediately of this change.
If you have insurance, do not be alarmed if the charges are higher than you are expected to pay according to the sliding scale. For example, the full cost of counseling service is $96 per hour and that is what the insurance company will be billed. You are expected to pay for service at the sliding scale rate until such time as your insurance company pays. If a payment is received from your insurance company and it is greater than the cost‐of‐service, then the amount you have paid in excess of cost‐of‐service will be refunded in full to you or applied to an outstanding balance. If the insurance company payment is less than the cost‐of‐service, you will be responsible for a co‐payment according to the fee schedule, up to but not exceeding the cost‐of‐service.
SLIDING SCALE
CTI offers a SLIDING SCALE fee payment for those qualifying according to income and family size. This means that professional services may be FREE or offered at a DISCOUNTED rate, depending on your income and family size. CTI is funded in part by the Oklahoma Department of Mental Health and Substance Abuse Services and the federal government. This means even if you have no insurance you can be eligible to receive services at our agency free of charge. Please contact a CTI office for more information about our sliding scale policy, a sliding scale application and to see if you qualify for discounted service rates or no-cost services. If we provide the services you need, you will NOT be denied services based on your inability to pay.
